The interface only specifies how to hand over a request but does not really prescribe how the fulfillment should be structured.
From a practical perspective, I believe, the fulfillment of individual order items from TMF641 is up to a SOM application. A client app (e.g. a COM) can provide additional information to suggest the execution process sequence (order item priority, dependencies and relationships) but the final decision on the orchestration process is still up to a SOM app

From my perspective the third option is the better.
When they are described the relationship (at order item or service level) has to be considered for the orchestration (accordingly to the relationship semantic).

- Sequential (one by one) execution of SOItem(s) or
- Parallel execution of SOItem(s) or
- Orchestrate based on serviceOrderItem + orderItemRelationship + serviceRelationship and rest of the SOItem(s) simultaneously.
