It's important to understand that eTOM is a taxonomy of business processes, with the different levels (L1, L2, L3, etc.) giving more and more details. But eTOM doesn't give an actual specification of the process flow itself (i.e. the steps in the process, the decision points, branches, etc.).
As Sharad wrote, you need to start by understanding your customer's needs, and finding the corresponding eTOM process elements that map into these business use cases.
But the actual process design will need to be done using an open source or off-the-shelf business process management tool that allows process design and runtime execution. It's not my place to give concrete recommendations, search the internet for BPMs and find customer references on their websites.

The simplest way is to start from what customer wants and how the company can deliver it. For example customer wants to "buy" and then start creating flow how eTOM enables the buy need of the customer. Then understanding it will be simple.
For implementation, start with mapping the processes to:
(1) Importance and priorities for the organization for each process components
(2) Team. Like WBS
(3) Automation (software, refer TAM - telecom application map and SID models.
